The diagnosis was confirmed through additional studies showing high opening pressure in the cerebrospinal fluid and papilledema observed by ophthalmologists, indicating that her vision was indeed compromised due to the increased intracranial pressure. Recognizing that this venous stenosis was the primary cause of her recurrent loss of consciousness rather than a heart problem, the medical team made the urgent decision to surgically open up the affected vein. This intervention required the support of the entire cath lab team and involved Dr. Sukulan Purokayastha and his colleagues to perform a groundbreaking procedure. The culmination of this complex journey was the successful stenting of the brain vein, marking the first time such an angioplasty had ever been performed on a vessel within the brain specifically for a case involving loss of consciousness without cardiac symptoms. This innovative approach not only addressed the root cause of Lalita's debilitating episodes but also highlighted the importance of looking beyond standard cardiac evaluations when syncope is accompanied by neurological signs like headaches and vision changes. She had recurrent episodes of loss of consciousness, persistent headache associated with the blurring of vision. So, obviously, the cause of syncope first which comes in our mind is the cardiac etiology. So, we evaluated her properly for coronary angiogram, recurrent halter monitoring reveals almost normal reports. Carotid angiogram, all the cardiac evaluation came normal. So, the person was frustrated. Why I am having still the syncope? So, we evaluated her for any kind of brain abnormalities like that because she was having headache to some extent persistent after 1 or 2 months of episodes. That's why the MRI brain reveals an empty sella. That is the pituitary gland was not present that time. Again, on investigating properly, we go deep into the matter. And with the help of Dr. Kaushik Dutta, our uh neuro medicine specialist, he gave us the opinion that we must investigate for MR venogram of the brain because the person or the female lady had persistent headache in spite of the almost normal MR brain. And the pituitary only absence will not cause this kind of headache. So, the MR venogram reveals a tight stenosis in the right sinus, transverse sinus of her brain. That is the blood vessels, veins within the brain had a significant stenosis. So, this is a ridiculous case we came to know. This kind of things may rise the pressure within the brain. So, we went for a CSF study. That means the brain's fluid study as well. The opening pressure and all those things was high. We had a ophthalmology opinion. The lady had a papilledema as well. That means her vision was also hampered. So, we immediately took decision to open up that vein. And this was probably the most important cause behind her recurrent episodes of syncope or loss of consciousness. And then we came to go for a definite investigation as well as the further proceedings for the operation with the help of Dr. Sukulan Purokayastha and my team. The whole cath lab team supported us. And we are able to stent that brain vein. That means that is the first time we have ever done an angioplasty within a brain vessel that too causing loss of consciousness without any cardiac symptoms.
