Hello everybody and welcome back. I'm
going to teach you the basics today of
microbiology. We're going to talk about
the enamel, the dentin, cementum, how
everything is formed, what cells are
involved, and all of that fun stuff. So,
this is actually taken from the dental
elm master book. I love it. It just
keeps things simple, everything
summarized if you're curious what you
are looking at here. So, this is all
included for board exam prep academy
members if you're studying for the board
exam, but you can purchase the master
book separately if you wanted to as
well. I can leave the link for you in
the description. Okay, so let's just
move right into it. So, we're talking
about enamel first. Okay, so when we're
talking about the tooth, the outermost
layer is going to be the enamel. But
then inside the enamel, you're going to
have the dentin. The enamel is white and
very strong. The dentin is going to be
kind of a yellow color and not nearly as
strong as enamel. Dentin is what's
exposed if somebody has sensitive teeth.
Typically, let's say somebody has
gingerville recession. Their enamel has
been either taken away from mechanical
forces or the gum has gone back. So the
dentin is now exposed the underlining
tooth structure. But then inside all of
that we have the pulp. We have the nerve
of the tooth. And cementum is what's
covering the root surface only. You will
not see cementum on the enamel. Cementum
is going to be underneath the gum line
but specifically covering the root
surface. So back to enamel here. So the
enamel organ comes from the ectoodderm.
What is the tissue type here? It is the
epithelial tissue type. Denton, for
example, is a different tissue type.
That's the connective tissue. So, when
you're studying microbiology
hisystologology for the board exam or
even if you're in the microbiology
hisystologology class in school, you're
going to need to know the different
tissue types. Okay? So, but what forms
enamel? What forms dentin? What forms
pulp? So, they're all different. So
enamel is formed by the amo blasts.
Dentin is formed by the odontol blasts.
Think o don d d d d d d d d d d d d d d

you to remember it. Amoblast kind of
sounds like enamel but starts with a.
Okay, so enamel obviously starts with E.
Amoblast starts with A. And then pulp is
formed by the fibro blast. So something
different entirely, right? So, but what
you need to know when enamel is being
taken away or building back up again,
that is done by certain cells. So, the
odontolasts
are going to take enamel away and
odontolasts
build enamel back up. Now, why would
enamel be taken away? Okay. So, when
we're talking about primary teeth, the
enamel could be starting to absorb a bit
along with the root when the teeth are
about to fall out. Okay. Um, you know,
teeth don't just magically disappear or
anything like that, but that can kind of
be something that as the tooth is
falling out. Now, what about the dentin?
Right? So, when we have the dentin,
remember those are going to be
odontolasts.
So odontolasts are going to take the
dentin away. Odonto blasts build the
dentin back up. So these are different
cells that help to either build or take
something away. But for the board exam
and probably for any test or exam you
have in school, you also need to know
the mineral content. So 96% is for
enamel and 70% is going to be for
dentin. um just kind of as an example,
those are the most popular ones that
they like to talk about. But then we
have cementum where the mineral content
is going to be 50%. Okay? And then we
have organic and water content. 38%
protein, 12% water. I'm just looking at
this from the master book. This isn't me
memorizing the different percentages.
I've been a tutor for 23 years and I
still can't memorize the different
percentages. So, in case you're
wondering where I'm reading it from,
it's just from the master book page that
I'm kind of looking at right here. So,
our enamel, dentin, pulp, and cementum
is going to be made up of all of these
fun things. Okay? And you just basically
need to know what cells are involved,
what the tissue type is, and that is all
here in the master book. Um, feel free
to take a picture with your phone if you
like, if that's if that's easier for you
to remember and study from. There's a
little picture here. Um, you know,
enamel, dentin, the pulp is in here and
then cementum is covering the root
surface. Now, moving down now, there's
more parts of the tooth and tooth area
we have to know. And that is the avular
bone and the periodontal ligament. These
are just as important because when we
talk to patients, we're not just talking
about the tooth itself. And patients
always have questions about teeth. They
always always do. You know, um tooth
tooth enamel is something that most
people know about, but the the the next
layer of the tooth, the dentin, people
don't know about. And they're going to
ask you, especially in the case of
gingerville recession, you know, why is
it yellow? Why does it look thinner?
Well, that's because the dentin's
exposed. Or they might ask you, why is
the tooth sensitive? Well, that's
because the dentin is exposed. But then
when you're talking about gum disease,
when you're talking about losing the
bone support around the tooth, when
you're talking about periodontal
disease, you're going to be talking
about the aviolar bone. And that's
produced from something different that
is produced from the dental sack. The
tissue type is going to be connective,
but now we're talking about the osteo um
the osteoblasts or osteoclasts.
So osteoblasts build the bone back up
and then osteoclass take the bone away.
Okay. Um and and actually I just
realized I misspoke above and I need to
fix this in the master book. I'm
surprised nobody's mentioned this to me
yet. Um I said here under enamel if you
look at resorptive cells so cells that
take things away for enamel it is not
odonto class it's going to be um amo
class because it's always going to have
to do with the formative cells. So for
enamel the formative cell is amo blast
building things up well taking it away
is going to be amo class not blasts but
class. So I have to fix this. For dentin
it's going to be odontoblast and odonto
clasps. Okay. For pulp it's going to be
fibroblast and fibro clasps. Okay. So I
do apologize. I'm going to fix this
later on. Um but then talking now. So we
did the cementum already. That's cement
blast cement clasps. Now we're done um
down to the avular bone again. So
continuing on we have the osteoblasts
and osteoclass. You know osteio refers
to a bone and the mineral content is
going to be 66%. The organic and water
contact is um content is 21% protein 12%
water. You do need to know that for the
board exam but when we're talking to
patients you can sound super smart you
know and start talking about osteoblast
osteoclass. This is something that will
come up in regular conversation. But
what about the periodontal ligament? So
this is also produced by the dental sac.
Um fibroblasts are the building up the
cells fibro class take things away. Um
but for the periodontal ligament um it's
going to remain intact. Okay. So we'd
like the ligament to be intact. Right?
That is I wonder is there a picture down
here? I know I have pictures inside the
board exam prep academy. This is more a
summary, but the periodontal ligaments
um you can't see my pointer either
unfortunately, but it's right around the
tooth. Okay, the perial ligaments start
to go away when there's periodontal
disease. So, when we're probing, there
should be a 1, two, or 3 millimeter
pocket in health. If it goes down to
four or more, we could be starting to
lose the periodontal ligaments,
especially five or above because that's
when you start to lose the bone support.
We want the periodontal ligaments to be
there. Okay, very very important. And
this is something we can talk to
patients about. Even when um patients
tell you, you know, um why is my tooth
still sensitive after a root canal? I
thought we took out the nerve of the
tooth. Well, critical thinking here is
to tell them, well, your ligaments
around the tooth could still be affected
and that can make things sensitive,
especially if there's bone um bone loss,
if there's periodontal disease
happening. So, terms to remember here.
So, I did do it properly here you guys.
Um um remember how we're talking about
um like odontoblasts odonto clasps
amoblast um amocclass o um osteoblasts
osteoclass. So these are just terms to
remember to kind of summarize everything
together. So you need to know which ones
are for avular bone osteoblast
osteoclasps which ones are for cementum
cement blast cement class. Make sure to
know those. They love to ask that on the
board exam. Now when we're looking at
different stages, okay, so this is
stages of tooth development. Alo blast
stages, the morphogenic stage, the
organizing stage, the formative
secrettory stage, the um the maturation
stage, the protective stage, and the
desmolitic stage. Okay, I'm probably not
not pronouncing these perfectly, but you
get the idea. So this the amolast stages
forming of enamel tooth development,
right? Forming the tooth. This is going
to be several pages in your textbook
that you're going to have to memorize,
right? I've made it in a summary for you
so much easier to study. This is what
you have to know for the board exam. If
you're studying the board exam, you
don't need to know 10 pages of the amo
blast stages. You need to know a big
paragraph like this one. And and
summarizing this, isn't this so much
easier? So the first stage is the
initial um initial stage, the
morphogenic stage. So this is initially
where the tooth germ undergoes changes
to form the shape of a future tooth. So
it doesn't look like a tooth yet, but
the shape is there. So the dental um
lamina proliferates and forms the enamel
organ, which surrounds the dental pill
and dental sack. So you need to know
what's first. The dental lamina is
forming the dental organ or um I'm sorry
the enamel organ which surrounds the
dental pill and dental sack. Now things
are becoming organized in the
organization um the organization stage.
So the enamel organ begins to organize
into different layers and these layers
you have to know. We have a full um
class just about the layers and putting
it together in the board exam prep
academy. I believe it's module 2. Um but
so it forms layers the outer enamel
epithelium, the stellate reticulum, the
stratum intermedi, and the inner enamel
epithelium. A must know. So we talked
about the dental pill in the first
stage. Now it's going to differentiate
into odontolasts.
Okay, odontolast, the dentin building
the dentin up. See, which later forms
dentin. The next stage the formative
secrettory stage this is where active
secretion of the enamel matrix proteins
by amoblast. So the amolasts are
building up the tomes processes
specialized extensions of amolast form
and deposit enamel matrix along
developing dentin. So in other words,
dentin or sorry, enamel is building up.
Amoblasts are the cells that build up
enamel and they're going along the
developing dentin surface. So the dentin
is developing. This is critical. The
enamel needs to go around the dentin,
right? The the strong enamel needs to
protect the dentin um layer and then the
maturation stage. So after this enamel
is formed around the dentin it now
shifts to maturing the enamel crystals.
So the enamel is forming but the
crystals the like making it even
stronger is now starting to form now. So
the amaloblasts those cells building up
enamel are allowing the crystals to grow
and mature. This is essential for making
the enamel hard. So all of this is
happening and what helps to make enamel
hard? um sodium fluoride or basically
any type of fluoride is going to
remmineralize enamel making it hard
which in my opinion is why fluoride in
the water is important. It's important
to get that systemic fluoride because
this is what's protecting the teeth,
making the teeth strong. Um calcium they
say makes the teeth strong as well. So
calcium sources that you're eating and
drinking makes enamel strong. Okay. Um,
and it doesn't just have to be milk. A
lot of people think that milk has the
most calcium, that cheese has the most
calcium. But what about somebody who is
a vegetarian or somebody who doesn't eat
milk or cheese because maybe they're
allergic to it, right? They don't have
horrible soft teeth. You can get calcium
from other sources. Also, systemic
fluoride in the in the water. But then
you have the protective stage, which
happens after tooth eruption. So now the
teeth are starting to erupt. The enamel
is in the oral environment. The amo
blasts form the protective layer called
reduced enamel epithelium which covers
the enamel surface to protect it from
acid and damage. So that's important
right called the protective stage. And
then the last stage the desolic stage
occurs during tooth shedding. So when
the the tooth is coming out the primary
tooth is coming out. This involves a
breakdown of the desmosal connections
between the amobol blast and enamel
services. This allows the enamel to
separate from amobolast and be shed
along with the primary teeth. So see how
all of these stages are so important.
They're summarized in the master book
for you. And guess what? Me explaining
it to you, I bet you understand it now,
don't you? You probably have to take
notes. You probably have to listen to
the video once more. But I guarantee you
after three times maybe of listening to
me teaching it, reading reading over it,
you're going to be a pro. This might not
be something you're talking to patients
about, right? But you understand it now.
How the teeth are formed. Okay? And
there's even more. Remember how we
started talking about the different
layers that are very necessary. There's
more involved too. Okay? So this is all
