Video summary

Dr. Jagannath Dixit introduces the DaVinci Xi system as the latest fourth-generation robotic surgical technology available in India, highlighting its three primary components: the patient cart, the vision cart, and the surgeon console. The patient cart features four slender universal arms, including a camera arm and three working instruments, which are mounted using an innovative vertical boom that allows for nearly 180-degree rotation. This unique design prevents the arms from colliding with each other and provides a clearance of 45 degrees, ensuring smooth operation even in tight spaces. The camera itself offers binocular vision with a range of motion extending 30 degrees up and down, equipped with halogen bulbs and telescopes to provide high-quality illumination and imaging during procedures. The vision cart serves as the central hub for monitoring and controlling the robotic arms, featuring a touch screen module that displays real-time data on all four instruments. It allows surgeons to adjust camera angles, mark or erase annotations, and view usage statistics, while also housing a high-end DaVinci E200 generator with various energy sockets for surgical tools. A key feature of this system is the fiber optic light source that delivers bright illumination directly into the surgical site. The console connects to these components via ergonomic interfaces, enabling precise control over instrument movements and energy settings, which are essential for complex minimally invasive surgeries. At the surgeon console, the doctor sits in a comfortable, ergonomically designed position that mimics natural hand movements, allowing for intuitive control of the robotic arms through master-slave technology. The system translates the surgeon's hand motions with varying ratios—ranging from 1.5:1 to 3:1—to enhance precision, particularly in confined areas where fine adjustments are critical. Controls include pedals for shifting and braking, as well as hand controls that offer different modes such as quick, normal, and fine, alongside advanced features like digital zoom, haptic feedback, and Firefly technology for enhanced visualization using indocyanine green (ICG) imaging. The robotic instruments replicate the full range of human wrist movements, including rotation, flexion, extension, and circumduction, with sharp scissors and bipolar tools that offer superior dexterity compared to traditional laparoscopic equipment. The surgeon wears an earpiece for direct communication and relies on a high-definition 3D display offering 10x magnification to navigate the surgical field with exceptional clarity. By integrating these advanced technologies, the DaVinci Xi system enables surgeons to perform intricate procedures with greater accuracy, safety, and efficiency, ultimately improving patient outcomes through minimally invasive techniques.

Hi dear friends, I'm Dr. Jagannath Dixit. I'm a surgical oncologist, lead consultant working at Manipal Hospital in Lanka. So today, you know, we are going to unique unique gadget called DaVinci Xi instrument. This is the latest available technology in India and it is the fourth generation, you know, technology. So the unique component of robotic as such, you know, has got a three important components. One what we call is the patient cart. On my right side is called the vision cart. And also on the corner there you can see the surgeon console. So [music] these are the three components of the robotic DaVinci component. So coming to you know here, you can see that you know these are all universal arms >> [music] >> and you know there are four arms. One camera comes and another two arms are the working and the other one is the the third arm. So these arms are very slender. And their unique is that of the Xi component is that the boom comes vertically and it makes a almost a 180° angle rotation. That is a unique part of that. You know, this docking [music] can happen next to the patient or it can come from the other side also. So coming to you know this important component, since the boom rotates almost like 180°, you don't have to worry about the you know the the clashing of this individual arms and there is a clearance of individual arms by 45°. Once you focus the camera, [music] so this is the unique component called as a camera here. You can see that this is a unique nature and it has got a you know mentioned here 30°. So this camera you can make a [music] 30 up and 30 down and this is what we call switching on the you know lamp here. Then taking [music] the still photos is all possible through this. So this is already written there is a 30° and another one is a 0° and it has got a binocular vision >> [music] >> and you can see that, you know, here is the, you know, the component of the two halogen bulbs comes and with you know, the two telescopes. [music] This is a unique feature of the camera. So, coming to the cannula, these cannulas are 8 [music] mm. So, it is easy to put it into the slot. You have to press it and it release like that. So, this is [music] how So, this is how we mount the cannula. If you look at the arms, individual arms, you know, these are all the very fine arms. You can see that this is a scissor. This you can see the how the rotation happens. So, this is how we put it into the slot. You know, I will show you this is how we put it into the slot. So, this sensors, they instrument and then, you know, all arm movements can happen. This is how the range of movements happens in different directions. So, coming to the boom as such, [music] here you can see that the, you know, boom is controlled here. So, this is a power button. So, these are all the, you know, different movements you can do. One the boom position, you know, then the height of the boom >> [music] >> and this is how we drive the car. So, this is the picture and you can see that these are all the different settings which [music] are available. So, coming to the um the vision card. Vision card, the unique nature is that it has got a touch screen module and where it it will display all the instruments in each individual arms, you know, like this, one, two, three and four. And here you can see that, Uh, you know, the what 30° up and down and the type of the camera and even you can mark it here and also you can erase it. And it will also shows the number of usage which are happened in the here in the display. So, this is what we call vision card and other additional component of the vision card you can see that this is energy source. [music] This is a very unique high-end available in the world as such DaVinci E200 generator. It has got two monopolar socket and two bipolar sockets. This is the where the neutral the card comes [music] out. So, this is where the camera, you know, attachment comes. This has got a beautiful light source [music] coming all the way into the through the fiber optic cable. So, these settings are available. So, this is what we call as the you know, the vision card. So, coming to here is the corner you can see that this is the surgeon console. This is the place where the surgeon sits comfortably and these are all the ergonomics are at at rest here only. You know, I can make the individual adjustment of the you know, the height and all that. This is the you know, the connections. This is a clutch pedal. This is a camera zoom in, zoom out. This is my bipolar. You know, right hand and this is my bipolar on the left hand or unipolar here and bipolar here. This is the vessel sealer or the stapling. If it is coming to right side, it will be the right or the left. I have [music] to you press this. And this is a shift pedal where you can align. And this is where my fingers [music] you can see here. It goes like this. And this is how you know, the movements happens at the [music] this level. So, the most important is that my head has to be placed then you know, this this movements becomes easier. So, and here you can see here. So, this hand control has got [music] three range, one quick, normal, and fine. So these are all the thing it moves 1.5:1, [music] 2:1, and 3:1 like that. So whenever you are doing in a very closed area, probably fine will be good. And so these are all the display, the type of haptic zoom is there, digital zoom is there, and work distance, then the right eye, left eye, then this is a tile pro, and the Firefly technology. ICG mode is the Firefly. This is a hand control we have seen. And this is the energy source which we have seen that you know you can adjust yourself. And this is a type of instrument. Now the arm one has got a monopolar curved scissor. Arm three is my right hand Maryland bipolar and the arm two will be the scope. Now I can [music] put you in ICG mode here and this is 30 up, 30 down. You can see this is a energy cord I can do the [music] settings. So this is the name of the surgeon you can use be displayed here. >> [music] >> And the surgeon and you know alphabetical order it will be there. If somebody wants to add account we can use there here. First name, last name, [music] and all that. So they can add that. So these are all the features this is available. This is a hand [music] control where you have to insert and move that. So these two fingers will go into the sockets here. This is the mid [music] index finger goes on the clutch here. So once you put the head like this and move, it will be translated here. I can make it this direction. I can go up, go down, and also circumduction all around. These are all possible. This is the scissor you can look at the sharpness of the scissor. And you know this is a unique nature. [music] You can see the bend how much I can do here. And range of movements and all that. I can make it this direction or I can [music] make it this direction. So both the instruments work together, you know. They are quite aligned. [music] So this sharp instrument preferably right hand, and the left hand will be the usual deal, our bipolar [music] source. So, this is the range of movement which I have got. This is a very, very beautiful, ergonomically very good, and almost all the human hand, how it rotates, [music] the instruments makes in the similar fashion. >> So, here you can see the inner vision, and this is [music] my ear piece where, you know, whenever and the these speaker phones are here, and whenever I give a command, it goes directly there. And this is my, you know, the the once I place the head, the instrument start moving. You can show the inner view here. So, that is this is a, you know, 3D 10x magnification, and you will have a beautiful immersive view >> [music] >> with a 3D background. So, this is about the surgeon console. Thank you. Thanks a lot. >> [music]
