Burn Injuries Explained: Types, Degrees & Life-Threatening Complications | Dr. Chintan Gujarathi
Watch on YouTubeVideo summary
Burn injuries can affect individuals of all ages, ranging from superficial sunburns to life-threatening deep wounds, making it essential to understand their classification and severity. The first degree is a very superficial burn similar to a common sunburn that typically heals within three to five days without scarring, often aided by topical creams or aloe vera. In contrast, second and third-degree burns involve partial thickness damage where healing times vary significantly from a few weeks to two months, frequently resulting in scarring. The most severe category is the fourth-degree burn, which penetrates deeply into the tissue and is visibly evident to the naked eye, requiring immediate and specialized medical intervention.
When it comes to seeking medical attention after a burn injury, time is critical as delays can lead to serious infectious complications. It is imperative for patients to consult a plastic surgeon immediately rather than waiting for a day or two, because only a specialist is in the best position to accurately evaluate the wound depth and determine the appropriate treatment plan. The approach varies depending on the specific type of burn, with partial thickness injuries often requiring specialized care to manage pain and prevent infection while ensuring optimal healing conditions.
For painful partial thickness burns, modern treatments utilizing collagen dressings offer significant advantages over traditional methods by eliminating the need for repeated daily dressing changes. This innovative surgical approach involves cleansing the wound until the bed is healthy and then applying an artificial collagen dressing that covers the entire injury site, allowing it to heal without further disturbance. The primary benefit of this technique is that patients avoid the pain associated with frequent dressing removal and reapplication, while also experiencing faster healing, minimal scarring, and better skin pigmentation as the underlying skin regenerates beneath the protective layer.
As the skin heals underneath the collagen dressing over a period of one to two weeks, the top layer gradually peels off naturally once the wound has fully epithelialized, leaving behind healthy tissue without the need for additional interventions. This method ensures that wounds heal uneventfully with good cosmetic outcomes, effectively addressing the pain and complexity associated with burn management. Ultimately, early consultation with a plastic surgeon combined with advanced dressings like collagen significantly improves patient recovery, reduces infection risks, and minimizes long-term physical and psychological impacts from burn injuries.

So three questions again. Number one,
what are the types or degrees of burn
wound and who are affected? Second
question is should the patients come
immediately or wait for a day or two
after burn injury and third is what are
these collagen dressings or surgeries
that we do as plastic surgeons in burn
wounds. So I'm Dr. Dr. Chintan Gujarati,
consultant plastic, cosmetic and
reconstructive surgeon at Manipal
Hospitals Kari.
First question is the most important
type, degrees and who all are affected
by burn injuries. So burn injury can
practically affect anyone from young to
old or the unlucky ones. Right? So it is
important to understand the degrees of
burn injury. So the first is a very
superficial burn. So it is kind of a
sunburn that most of us must have
experienced. These sunburns usually heal
without any treatment also within 3 to 5
days for which any topical creams or
aloe vera lotions etc. help. The second
and third degree burns are those burns
which are partial thickness depending on
the depth of the injury to the skin
wherein they might heal within 3 to 5 or
7 days or might take 3 to 8 weeks as
well to heal with scarring. And the
fourth degree burn is of course a very
very deep burn which is evident to the
naked eye as well. Second question is
whether to report to the surgeon or
doctor immediately or wait for a few
days to come. So time is critical in a
burn wound. As the delay happens, the
delay more you wait to come to the
surgeon or doctor more are the infective
complications. So it is extremely
important
plastic surgeon
then only the plastic surgeon is in a
better position to evaluate the burn
wound and treatment also varies
according to that burn
collagen dressings burn surgeries. So
whenever there is a partial thickness
injury in a burn
painful partial thickness
treatment collagen dressings now what is
the beauty of this surgery is no
repeated dressings are required no daily
dressings no pain of surgery and the
wounds heal faster and much uneventfully
with good pigmentation as well. So
basically,
you cleanse all the burn wounds nicely
and then once the bed is healthy, we put
a artificial dressing called as a
collagen dressing over all the wounds.
So this covers all the wounds and we
need not touch that wound or dress that
wound repeatedly. That is the beauty. So
directly after maybe a week or two as
the skin below the collagen dressing
heals and it epilizes gradually the top
layer of collagen gradually comes off
and the wounds heal without any
dressing. Thank you.
