Breast Cancer Treatment & Recovery | Dr. Rajshekhar C. Jaka | Manipal Hospitals Old Airport Road
Watch on YouTubeVideo summary
The video addresses the diagnosis and management of a stage two breast cancer case, emphasizing the critical importance of evaluating any breast lump promptly. Upon discovering a lump, the medical team performed a core biopsy which confirmed the presence of carcinoma. To determine the appropriate treatment strategy, immunohistochemistry tests were conducted to assess specific markers including ER, PR, HER2, and Ki67. The results indicated that the patient had a low-risk tumor characterized by positive ER and PR status, negative HER2, and very low Ki67 levels, suggesting a well-behaved cancer with a favorable prognosis.
Given the nature of the tumor, the focus shifted to surgical options aimed at preserving the breast whenever possible through conservative surgery or mastectomy. In this specific instance, the patient chose a mastectomy over breast-conserving surgery because the latter would have necessitated radiotherapy, which she wished to avoid. The transcript clarifies that while breast conservation combined with radiation offers outcomes comparable to mastectomy, the decision was driven by the patient's preference to spare herself from radiation therapy, highlighting the role of patient choice in surgical planning alongside medical necessity.
Following the surgery, the recovery process proceeded smoothly, and the treatment plan primarily involved hormonal therapy administered as a simple tablet that the patient could manage at home. This medication is typically taken for the initial two years, followed by periodic checks every six months up to a five-year follow-up period. The video concludes by underscoring the significance of these long-term follow-ups, ensuring that any minor issues or changes are addressed promptly to maintain the patient's health and well-being throughout her recovery journey.

Now this patient came with the history
of lump in [music] the breast. It's very
important that any lump in the breast
needs to be evaluated. Luckily they came
when they noticed the lump and we did a
core biopsy which proved that it is a
carcinoma or a cancer of the breast.
Then it is very important nowadays to do
IHC iminohistochemistry that is ER, PR,
Honu and key67. [music] These four tests
are very important on each case of
breast cancer. So we did did that and
luckily it was found that she had a low
risk tumor that means E er PR is
positive R2 is negative and key67 is
very low [music] that means it's a very
well- behaved tumor. So in that case the
surgical options takes the priority and
there are two types of surgery where we
preserve the breast which is known as
breast conservative surgery or we remove
the whole breast that is known as
mastctomy.
Uh in her case they opted for mastctomy
because whenever we conserve the breast
it is for the cosmetic reason and they
require a radiotherapy so they didn't
wanted radiotherapy for that reason we
did a mastctomy. So [music] breast
conservation radiation both together
equals to mastctomy.

Post surgery the recovery was very good
and most probably we will be giving her
only hormonal treatment which [music] is
just a simple tablet which she can take
at home uh in the initial 2 years and
later on once in 6 months up to 5 years
follow-up is very very [music]
important. So we any minor things we
will take care of during the followup.
Thank you.
